Initial Consultation Form

During an initial consultation, an attorney from Kulwin, Masciopinto & Kulwin, L.L.P. (KMK) will discuss your legal matter with you, including the possibility of pursuing legal action and the likely costs involved.

It’s important to note the purpose of the consultation is not to give a definite legal opinion about your matter, because a matter cannot be fully assessed within the limited time of the consultation or with the information you can provide.

After the consultation, one of three outcomes is possible: (1) You and KMK mutually agree to the terms of representation and a separate document called “Attorney Client Agreement” is reviewed and signed by you and KMK; or (2) You decide not to use KMK’s services; or (3) KMK declines representation
